Telehealth referral process under CDMP

1. G.P. Referral

Patients in category 3 regional areas may be eligible for bulk billing under GPCCMP. Once you assess your patient requires referral for dietitian services , you can forward your completed referral form to our clinic via:

For patients not eligible under GPCCMP, we can also work with them privately towards their health goals via telehealth. See our fees for private dietetic services.

2. Nominate services

We recommend nominating at least 3 dietitian service appointments, for the best result. Some complex cases may require all 5 services. At any stage, you are welcome to reach out to discuss this aspect prior to referral.

3. Book appointment

If you choose to, you may also book your patient’s first appointment for them via our online booking system. We offer early and late appointment options on nominated days over phone, Facetime, or Zoom at patient’s request. All appointments for category 3 rural patients referred under GPCCMP/CDMP are bulk billed with no gap fee payable. The current Medicare rebate of $61.80 per session applies to GPCCMP patients outside of category 3 regional areas.

A copy of the GPCCMP is required prior to the appointment to avoid full fees. You may also like to provide your patient with our forms prior to their appointment.

Appointment structure under GPCCMP

We recommend at least 3 appointments, which will usually be scheduled fortnightly.

First appointment: Understanding the patient

Experience shows that patients trying to follow cookie-cutter solutions can experience overwhelm and disappointment. Our first appointment centres around understanding the needs of the patient, their goals, and the priorities we need to focus on. Once we understand this, then we conclude the session by providing resources the patient can work on right away.

Second appointment: Creating a tailored solution

The second appointment is about tailoring an accurate and appropriate solution or meal plan to suit the patient. Working with their lifestyle and preferences, and considering any observations made around previously supplied resources. It’s about creating a tailored solution that the patient is invested in, because it works with them and for them.

Third appointment: Review + habits for life.

We review the patient’s progress, and make changes where required. This appointment also has an educational component. The aim is to arm patients with ways of thinking and practical tools that help them feel confident, and manage real-life situations with ease. This promotes the adoption of dietary changes as a new lifestyle over the long term.

Additional appointments: When are they indicated?

For complex issues, some patients may require additional appointments in order to diagnose and create sustainable and impactful solutions. In this case, 5 visits may be optimal. Within a 5-visit plan, we can also recommend exercise physiologist and psychologist services within our practice if this is of greater benefit to the patient than dietetic services.
